    YTD medical billing for my wife and I is $108,309. Our cost after insurance is $7379 plus ~$4200 in premiums. That's 7 months.

    So a little more than 10% is your OOP? That's not horrible at least.

    Correct, so for the balance of the year, I have another $3600 in premiums and another $1200 OOP on me until we max the plan - she's already maxed out. But I am still not sure what counts toward the OOP max. I think copays are excluded?

    Yeah, if it's anything similar to my plan (I have United Healthcare), copays don't count towards your OOP expenses. I think that's the only cost that's excluded and everything else you pay goes towards what your max OOP is.

    Easy. The insur8co knows you're going to drive the van more, and, if the van is insured commercially, pretty sure the minimum coverages are higher.

    nope, same estimated annual milage and non commercial use. Insurance works differently over here, there is no minimum coverage so to speak, just differences in how your own vehicle is covered an both policies were identical.
