Who's your health insurance through? I've never heard of anything like that at all

Wife and I both have United Healthcare through our employers and we've never been charged anything like that when either using regular office visits or urgent care.

I have United healthcare as well. They say it's not their problem, they can only process as billed.

Can you file a complaint with the state AG? it sounds like it's at best, shady billing practices, and at worst, illegal billing practices.

I requested a billing review. It has something to do with whether your doctor is at a hospital facility or a separate clinic. In the previous incident at a different hospital network, they had moved all facilities under the hospital billing unit for the sole purpose of extending Joint Commission accreditation to all satellite offices. Once they did that, the billing changed to include hospital facility charges. I called bullshit and they dropped the charges.
